Insurance, Billing, Referrals, and Prescriptions

 

Insurance

We participate with most insurance plans. To ensure that your plan participates with us, call the member services number on the back of your card to verify participation. You are responsible for any co-pay due at the time of service.  If we do not participate with your insurance company, you will be responsible for the additional out-of-pocket expense associated with visits to an out-of-network provider.

 

We will submit your claim to your insurance company for you, regardless of whether we are a participating network provider or not.

 

 

Billing

Payment for uncovered balances is expected within 30 days following insurance payment.  It is your responsibility to call our billing department and speak to an account representative if you are unable to make the payment.

 

 

Referrals and Pre-Certification

Referrals are governed by your insurance company. If your insurance requires a referral, please contact your primary care physician prior to your visit to our office to obtain the needed paperwork.

 

You may contact our office if you need assistance with this process.  We will help in any way possible.  However, your insurance is your responsibility and without proper authorization your visit may not be covered by your insurance.  In this case, you will be responsible for the cost of services.
 


Hospitalization
Certain conditions require hospitalization to be properly diagnosed or treated. In that event, our office will contact the hospital to coordinate your admission. Should you need to cancel or change your hospitalization, please contact our office first.

 

 

Prescriptions and Refills
When you need a prescription refilled, we ask that you call your pharmacy about one week before you run out of medicine. In the event you have no refills remaining on your prescription, your pharmacist will contact the appropriate office to obtain additional refills for you. If for any reason you need to call our office for a prescription, we need a minimum of 48 hours to handle your request. In some cases, you may be asked to make an appointment to see one of our physicians in order to refill your prescription.
